29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use.    On or about May 28, 2013, employees were exposed to a fall and struck-by hazard while working from a pallet that was being elevated on the forks of a lift truck.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   On or about May 28, 2013, at least one employee standing on a pallet without fall protection was lifted to the roof of a building on the forks of a lift truck and was exposed to an unprotected fall of more than 18 feet to the concrete below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.   On or about May 28, 2013, the employer did not ensure each employee was trained to recognize and avoid fall hazards to minimize those hazards. Employees performing work on a roof were exposed to an unprotected fall of over 18 feet to concrete below.
