29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :    a)  The company had not performed the annual audiometric testing for the blaster since November 8, 2011.  The blaster was monitored and determined to still be exposed to continuous noise at an 8 hour time weighted average of 92.2 dBA over the 435 minute sampling period on June 5, 2013 with a zero increment for the 45 minutes not sampled.    Neman Painting & Sandblasting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95(c)(1) which was contained in OSHA inspection number 122393028, citation numer 1, item number 1, issued on October 26, 2011, which was affirmed as a final order on April 30, 2012 with respect to a workplace located at 1405 Anoka Avenue Waukesha, Wisconsin 53186.

29 CFR 1910.95(k)(2): The training program was not repeated annually for each employee included in the hearing conservation program:    a)  The baster was not provided the annual training on the hearing conservation program and the effects of noise since December 7, 2011.  The blaster was determined to be exposed to continuous noise at an 8 hour time weighte average of 92.2 dBA over the 435 minute sampling period on June 5, 2013 with a zero increment included for the 45 minutes not sampled. 29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:    a) An employee in the production area used an elastomeric tight fitting half mask respirator with facial hair that would interfere with the seal of the unit to provide adequate protection.
