29 CFR 1926.451(f)(6):Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in this section, for example closer than ten feet to exposed and energized power lines having a voltage of less than 50 kv:    (a) 20 Cargill St Worcester, MA: On or about May 28, 2013, an employee was observed working from a metal pump jack scaffold system that was less than 6 feet from the energized power lines serving this residence, which is not in accordance with this subpart:    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:    (a) 20 Cargill St Worcester, MA: On or about May 28, 2013, an employee was observed working from a pump jack scaffold system that was more than 20 feet above the ground below without having any fall protection on the open end he was working from, which is not in accordance with this subpart:     Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
