29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  (a) 386 Elm St East Longmeadow, MA: On or about May 28, 2013, employes were observed working on a roof, more than 10 FT above the ground below, to side this residence without any fall protection as required by this subpart.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  (a) 386 Elm St East Longmeadow, MA: On or about May 28, 2013, employees were observed using a ladder to exit the roof of this residence that did not extend 3 FT past the roof surface as required by this subpart.
