29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Americarb, Inc., Ashland, Ohio: On or about May 28, 2013, the employer did not ensure that employees utilized energy control procedures prior to performing servicing and maintenance activities on the Haas 510 router machine. Employees were exposed to laceration and amputation hazards while engaged in activities, such as, but not limited to job set-ups and tool changes. b) Americarb, Inc., Ashland, Ohio: On or about May 29, 2013, the employer did not ensure that employees utilized energy control procedures prior to performing servicing and maintenance activities on the Omax 55100 Jet Machining Center. Employees were exposed to laceration and amputation hazards while engaged in activities, such as, but not limited to job set-ups. Americarb, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 313787079, citation number 2, item number 1 and was affirmed as a final order on May 25, 2012, with respect to a workplace located at 1025 Faultless Drive, Ashland, Ohio.
