29 CFR 1926.300(b)(2): Rotating or moving parts of equipment which were exposed to contact by employees or otherwise created a hazard were not guarded: At worksite, East Side: On or about May 29, 2013, employees were exposed to a caught-in hazard from a Tsurumi A.C. Generator (Model TPG-2900H-DX) where the recoil starter cover was missing, therefore exposing the rotating engine cooling fan blades.

29 CFR 1926.404(e)(1)(i): Conductors and equipment rated 600 volts, nominal, or less were not protected from overcurrent in accordance with their ability to safely conduct current: At worksite, East side: On or about May 29, 2013, an employee was exposed to a fire hazard from a Tsurumi A.C. Generator (Model TPG-2900H-DX) which was missing the 18 Amp circuit breaker equipped by the manufacturer and was therefore lacking overcurrent protection.
