Worksite located at 318 Walnut Street, Cameron, Missouri- An employee was allowed to work in an unprotected trench in which water was accumulating and adequate measures were not taken to protect the employee from cave-ins hazards.

29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face:   (a). Worksite located at 318 Walnut Street, Cameron, Missouri- An employee was not wearing a hard hat while loose rocks and debris materials were rolling into an unprotected trench. The employee was exposed to head injuries and other bodily injuries if hit by falling debris.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):   (a). Workplace located at 318 Walnut Street, Cameron, Missouri- The employer failed to provide an adequate protective system to prevent cave-in hazards. An employee was allowed to work in an unprotected trench that was not benched, sloped or shielded.

29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety:  (a). Workplace located at 318 N. Walnut Street, Cameron, Missouri- The competent person on the jobsite allowed an employee to enter an unprotected trench in which the soil was previously disturbed, water had accumulated from the sidewalls, spoil pile was located directly on top of the sidewalls, and the trench was exposed to vehicular traffic and vibration.
