29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: A) Avon Truck Service Center, Inc., located at 38525 Chester Rd., Avon, OH 44011: On or about April 29, 2013, the employer did not ensure that employees, performing vehicle maintenance such as, replacing tires, wore eye protection such as safety glasses. Employees were exposed to eye injury hazards from flying particles while inflating tires with air. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: A) Avon Truck Service Center, Inc., located at 38525 Chester Rd., Avon, OH 44011: On or about May 29, 2013, the employer failed to establish a lockout / tagout program for the control of the hazardous energy, which includes procedures, training, and periodic inspections, while performing activities such as, but not limited to, engine maintenance of the vehicles such as, but not limited to, International 4200 (Engine VT 365). It created a potential for the engine to be started while engine maintenance is being performed, exposing employees to laceration and amputation hazards. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
