29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     On or about May 23, 2013, during construction activities including but not limited to roofing work, the employer did not ensure employees were protected from possible head injuries by ensuring head protection was provided and worn by employees. An employee was unloading debris from roofing carts that were being dumped into a dumpster with the assistance of a crane.

29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe:    On or about May 23, 2013, during roofing activities including but not limited to removing roof debris and unloading material from upper levels, the employer did not ensure all rigging was inspected prior to use and removed from service when found defective. Rigging was being used with missing self-closing latches.
