On or about July 9, 2013, at the above addressed jobsite.  An employee was exposed to struck-by and caught-by hazards while driving a powered industrial truck without being trained in one of the power industrial trucks (Raymond truck) which were used at the jobsite.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about May 30, 2013, at the above addressed jobsite.  a.  An employee was exposed to an amputation hazard while working with a drill press (Jet Equipment & Tools, Serial number 6090188) that lacked point of operation guarding.  b.  An employee was exposed to an amputation hazard while working with a drill press (Jet Equipment & Tools, Serial number 6110099) that lacked point of operation guarding.  c.  An employee was exposed to an amputation hazard while working with a table saw (Jet Tools, Model JWTS-10) that lacked a guard over that portion of the blade above the table and the material being cut.  d.  An employee was exposed to an amputation hazard while working with a circular saw (Dewalt, 16-inches) that lacked a guard for cover the lower portion of the blade.  e.  An employee was exposed to an amputation hazard while working with a circular saw (Dewalt, 16-inches) that was not adjusted to limit its forward travel.

An employee was exposed to an amputation hazard while working with a mechanical power press (EW Bliss Co.) that lacked point of operation guarding.  c.  An employee was exposed to an amputation hazard while working with a mechanical power press  (Rouselle No. 2E) that lacked point of operation guarding.  d.
