29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) Estates at Borden's Crossing, 50 Crescent Drive, Bordentown, NJ 08505: On or about May 22, 2013, employees were not trained in recognition and avoidance of hazards associated with scaffolds. An employee was exposed to fall hazards while standing on a sheet of plywood that was elevated by a Sky Trak 8042 Telescopic Forklift. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
