On or about May 30, 2013, employer did not establish and implement lockout/tagout program for employees to receive training and utilize formal lockout/tagout procedures for energy isolation on the machines where the hazard of unexpected energization could occur and injure employees during servicing and maintenance tasks.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about May 30, 2013, an employee operating Milltronics VM30IL CNC was exposed to laceration, struck-by, caught-in/on hazards when reaching into the danger zone to take-out/place-in parts, and clean, where the safety interlock was not affixed as constructed and disabled during normal production operation.  b) Production Dept. West wall. On or about May 30, 2013, employees operating Milltronics VM24 CNC were exposed to laceration, struck-by, caught-in/on hazards when reaching into the danger zone to take-out/place-in parts, and clean, where the safety interlock was not functioning as constructed and designed during normal production operation.  c) Production Dept. North-west wall. On or about May 30, 2013, an employee operating Milltronics VM16 CNC was exposed to laceration, struck-by, caught-in/on hazards when reaching into the danger zone to take-out/place-in parts without sufficient safeguarding provided during normal production operation.  Abatement documentation is required for this item in accordance with the requirements of 1903.19(d).
