29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     The Denver Marble Company, at 1011 S Huron Street, Denver, CO: On and preceding 5/30/13, The Denver Marble Company did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles.  Employees used a Flow water jet, a GMM Gravellona Toce bridge saw, and Alpha Professional Tools pneumatic polishers to fabricate granite countertops.  These processes generated flying particles.  An employee operating the GMM bridge saw wore non-safety prescription eyeglasses.  The employer did not ensure that the employee wore appropriate eye protection.  This condition exposed the employee to a flying particle hazard.    The Denver Marble Company, at 1011 S Huron Street, Denver, CO: On and preceding 5/30/13, The Denver Marble Company did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles.  Employees used a Flow water jet, a GMM Gravellona Toce bridge saw, and Alpha Professional Tools pneumatic polishers to fabricate granite countertops.  These processes generated flying particles.  An employee operating the Flow water jet wore prescription safety eyeglasses without side protection.  The employer did not ensure that the employee wore appropriate eye protection.  This condition exposed the employee to a flying particle hazard.

Employees worked with chemicals during fabrication of stone countertops.  The employer did not have Material Safety Data Sheets for all chemicals in the workplace.  Employees worked with, but not limited to, the following chemicals:  1.  Standoff Stain Barrier; 2.  GranQuartz Pro Series Transparent Knife Grade Polyester Stone Adhesive; and 3.  Wood & Stone Company Acrylic Super Penetrating.
