Circuit breaker panel on third floor just below mezzanine level had an opening that was not guarded or covered to prevent inadvertent contact.

Electrically powered hand-tools (e.g. circular saw and drill) were not connected to a ground fault circuit interruptor; on or about 30 May 2013.
