29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.       On or about May 30, 2013, on the west side of the work site, employees were working without hardhats below other employees engaged in metal roofing activities.  The employees were exposed to struck-by or flying objects.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.      On or about May 30, 2013, on the west side of the construction site, employees performing roofing activities on the steep roof of a commercial building without fall protection were exposed to a fall hazard of approximately 25 feet to the lower level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.       On or about May 30, 2013, on the west side of the construction site, employees performing roofing activities on a commercial building utilized an extension ladder that did not extend 3 feet above the landing.
