29 CFR 1910.132(a): Personal protective equipment was not used and/or maintained in a sanitary and reliable condition when necessary whenever hazards capable of causing injury and impairment were encountered:  a/ Soiled utility room 300 Hall,  Soiled utility room 100 Hall, and Soiled utility room Special Care Unit; personal protective equipment including but not limited to eye protection and gloves were not present and used by CNAs and nurse aides who rinsed out soiled linen in the hopper.    b/ Kitchen, eye protection was not present and used by employee(s) who used corrosive chemicals to clean the oven.  c/ Laundry, eye protection was not present and used by employee(s) who changed out corrosive chemicals.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body was not provided within the work area for immediate emergency use:   a/ In the kitchen for employees who used corrosive chemicals including but not limited to Oven and Grille Cleaner (potassium hydroxide), HWD (Hard Water Machine Dish Detergent - potassium hydroxide and sodium hypochlorite), and Blast Heavy Duty Cleaner (potassium hydroxide), an approved eye wash was not available for immediate emergency use.  b/ In the laundry for employees who used corrosive chemicals including but not limited to 33- Blitz and DeStainer (sodium hypochlorite), an approved eyewash was not available for immediate emergency use.
