29 CFR 1910.106(d)(4)(i): "Construction." Inside storage rooms were not constructed to meet the required fire-resistive rating for their use. Such construction did not comply with the test specifications set forth in Standard Methods of Fire Tests of Building Construction and Materials, NFPA 251-1969. Where an automatic sprinkler system is provided, the system not designed and installed in an acceptable manner. Openings to other rooms or buildings were not provided with noncombustible liquid-tight raised sills or ramps at least 4 inches in height, or the floor in the storage area were not at least 4 inches below the surrounding floor. Openings were not provided with approved self-closing fire doors. The room was not liquid-tight where the walls join the floor. A permissible alternate to the sill or ramp was not an open-grated trench inside of the room which drains to a safe location. Where other portions of the building or other properties are exposed, windows were not protected as set forth in the Standard for Fire Doors and Windows, NFPA No. 80-1968, which is incorporated by reference as specified in Sec. 1910.6, for Class E or F openings. Wood at least 1 inch nominal thickness was not used for shelving, racks, dunnage, scuffboards, floor overlay, and similar installations:  On or about 05/22/2013, the employer did not ensure that the ink storage room was constructed to the applicable fire protection regulations in place when the room was designated as the ink storage room. The room contains various flammable and non-flammable inks and chemicals, such as but not limited to: RD9V, D-Glazer, and Prisco Scratch Remover.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  On or about 05/22/2013, the employer did not develop specific energy control procedures for the various printing, binding, and mailing machines with electrical and kinetic energy sources, located throughout the production facility, such as but not limited to, shutting down, isolation, securing, placement and removal of Lock-out/tag-out (LOTO) devices, testing of the the equipment to ensure effectiveness of LOTO devices, etc. Specific procedures will ensure that there should be no unexpected start-up of the equipment.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about 05/22/2013, the employer did not ensure that Lock-out/Tag-out procedures that were being used were inspected and verified as adequate for the various types of printing, binding, and mailing machines, on a annual basis. Not having a annual inspection of the LOTO procedures (documented or undocumented) allows for the verification of proper procedures and the identification of improper procedures that are being used.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  On or about 05/22/2013, the employer did not ensure that circuit breaker panels were not blocked to allow easy access to the panels. Circuit breaker panels at the locations below were blocked by equipment and material which would prevent employees to access the panels.  INSTANCES:      a) Circuit breaker panel behind the Heidelberg offset printing press was blocked by material placed in front of it.     b) Circuit breaker panel at gumming station blocked by large metal table positioned in front of the panel.     c) Circuit breaker panel in corner at end of gumming station blocked by material placed in front of it.      d) Circuit breaker panel at Jefferson Highway warehouse in Binding area was located behind the binding machine which requires         employees to go through and around the machine to access the panels

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.  On or about 05/22/2013, the employer did not ensure that flexible power cords were not protected from strain applied to the cord. A power cord from the OIO envelope Printer in the Digital Printing Dept was connected to a power strip, and due to the length of the power cord, held the power strip suspended approximately 1 ft above the floor.
