29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Quality Ready Mix, Inc. on or about May 28, 2013, at the address of 310 Quarry Dr. Bluffton, OH: The employer failed to assure an employee tasked with verifying the remaining contents of a storage tank positioned on an unguarded elevated surface was protected from a fall. The employee working from this surface in this condition was exposed to an approximant 17 ft. fall to the concrete level below.
