29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. 1. In the production area, at the time of the inspection, the employer was operating several multiple energy source (electric and pneumatic) machines (equipment) without a machine specific energy control (lockout/tagout) program in place.

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. 1. Inside of the warehouse dock area, employee had been using a Lasko thirty (30) inch pedestal fan that was not grounded. The metal casing (framing) on the appliance was energized and the ground pin was missing from the plug on the electrical (extension) cord used to energize the fan.

29 CFR 1910.212(b): Anchoring fixed machinery. Machines designed for a fixed location shall be securely anchored to prevent walking or moving. 1. Inside of the maintenence shop, employees were using a Milwaukee Heavy Duty chop saw that was not anchored to the metal table to prevent movement. Employees were exposed to caught in and struck by hazards.
