OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to fall hazards:     a. At the worksite located at 3010 Columbus Lancaster Road in Lancaster, Ohio an employee was fatally injured from a fall when he worked from a roof, approximately 20 feet in height without receiving authorized person training for the employee to recognize or control hazards associated with falls.
