29 CFR 1910.305(g)(2)(ii): Hard-service cord and junior hard-service cord No. 14 and larger were repaired by splicing, which did not retain the insulation, outer sheath properties, and usage characteristics of the cord that was spliced:Note: Hard service flexible cords No. 12 or larger may be spliced if the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. At this location: Employees engaged in sandblasting activities inside a steel confined space were exposed to shock hazards from contact with an flexible energized cord used to power a portable light fixture; and whose splice was repaired with duct tape. The portable electrical cord with splices covered with duct tape was a size 16-3; with an SJO insulation type, operated at 120 volts AC, and was resting upon the steel floor of the confined space.

29 CFR 1910.146(f)(3): The entry permit that documented compliance and authorized entry to a permit space did not identify the date and the authorized duration of the entry permit: At this location: The second half of the two-part entry permit record used daily for employees engaged in permit space sandblasting activities did not contain the date or authorized duration of the entry permit. The second half was used to verify monitoring of acceptable entry conditions taken at 15 minute intervals; as well as document the two signatures which accounted for all hazards removed from the space, and the cancellation of the confined space permit.
