29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   The employer does not guard the rotating parts of machines whose operation exposes employees to injury.   a. This violation was observed on or about May 31, 2013, at the facility where employees were exposed to struck-by and caught-by hazards when operating a Daewoo Puma 10 HC w/ Mitsubishi Control (1996) CNC Lathe with Serial #: PH100732 without the interlock for the door guards.   b. This violation was observed on or about May 31, 2013, at the facility where employees were exposed to struck-by and caught-by hazards when operating a Daewoo Puma 12L-C w/ Funac Control (1996) CNC Lathe with Serial #: PL120704 without the interlock for the door guards.   c. This violation was observed on or about May 31, 2013, at the facility where employees were exposed to struck-by and caught-by hazards when operating a Doosan Lynx 220LM 3 Axis Turning Center w/ Live Tooling CNC Lathe with Serial #: L220M0771 with the interlock for the door guards disabled.      d. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all machines that have exposed rotating parts are guarded.

29 CFR 1910.242(b): Compressed air, that was not reduced to less than 30 p.s.i., was used for cleaning purposes without effective chip guarding and personal protective equipment.  The employer does not ensure that compressed air used for cleaning is reduced to less than 30 psi. This violation was observed on or about May 31, 2013, when employees throughout the facility were exposed to struck-by hazards while blowing off equipment with compressed air that was set to a pressure of 130 psi.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all compressed air used by employees to blow off equipment is reduced to less than 30 psi.

29 CFR 1910.303(b)(1)(viii): Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment was not determined using the following considerations: Other factors that contribute to the practical safeguarding of persons using or likely to come in contact with the equipment.  The employer does not ensure that the electrical equipment was free from hazards that were likely to cause death or serious physical harm to employees.  This violation was observed on or about May 31, 2013, in the facility where employees were exposed to an electrical hazard from exposed live wiring used to power a conveyor.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that electrical equipment are free from hazards that are likely to cause death or serious physical harm to employees.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein.   The employer does not ensure that containers of hazardous chemicals in the work place are labeled with the identity and appropriate hazard warnings of the chemicals they contain. This violation occurred on or about May 31, 2013, when employees in the facility were transferring chemicals into spray bottles and storing them in their work area without labeling them with the identity and appropriate hazard warning of the chemical.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that all hazardous chemical containers, not intended for immediate use, are labeled with the identity and the appropriate hazard warning of the chemical they contain.
