29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:   a) for the 4-inch by 2-inch hole in the landing at the top of the entry way at Green Ohana Recycling located at 155 Sand Island Access Road in Honolulu (HI), as observed on or about 06/05/2013 and 06/10/2013.  The landing, which was constructed of metal, was severely corroded and had already developed a hole in front of the doorway into which employees could easily step.  Due to the amount of corrosion seen from the underside, employees will likely be falling  (39-inches) through the hole in a very short amount of time resulting in foot and leg injuries.

29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails:  a) for the stairs/steps at the entry of Green Ohana Recycling located at 155 Sand Island Access Road in Honolulu (HI), as observed on or about 06/05/2013 and 06/10/2013.  The railing for neither the stairs nor the landing included a mid-rail to prevent employees from falling thorugh the openings.  A fall from the landing (39-inches) could result in leg and body injuries to the employees.

The outlet was tested and found to be without a ground exposing employees of Green Ohana Recycling to potential electrical shocks.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:   a) for employees repairing pallets at Green Ohana Recycling located at 155 Sand Island Access Road in Honolulu (HI), as monitored on or about 06/10/2103.  On this date, during the 288 minutes of personal noise dosimetry, the employee's  8-hour time-weighted-average (TWA) dose was 83% of the allowable 85 decibels A scale (dBA).  The equivalent noise level for this dose is 89 dBA.  Zero exposure was considered for the remaining 192 minutes of the employee's 480 minute workday.
