29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:     a) Worksite, East side - Employees were using a 4 inch, square, steel quad junction box to power a circular saw, and charge a battery. The 4 inch steel double gang box was not mounted in accordance with the manufacturers instructions; on or about 05/31/13.     Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:    a) Interior, Southeast Corner - Employees were using an outlet to power items such as but not limited to an extension cord, powering a circular saw and a battery charger. The outlet did not have a faceplate on it; on or about 05/31/13.     Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
