29 CFR 1926.701(f): Employee(s) were applying a cement, sand, and water mixture through a pneumatic hose and were not wearing protective head and face equipment: (a) Little Mossey Tunnel near Pax, WV: An employee was standing next to his employer in the manbasket of a JLG lift during a shotcrete operation where a cement mixture was being sprayed onto the tunnel walls and ceiling. The employee was not wearing a faceshield, as documented on May 30, 2013. ABATEMENT VERIFICATION IS REQUIRED FOR THIS ITEM.

29 CFR 1926.50(g): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) Little Mossey Tunnel near Pax, WV: An employee was working in close proximity to an operation where a cement mixture containing corrosive Eucon Sureshot Shotecrete Accelerator was sprayed onto the tunnel walls and ceiling and a suitable facility to flush the eyes was not available as documented on May 30, 2013. ABATEMENT VERIFICATION IS REQUIRED FOR THIS ITEM.
