29 CFR 1926.502(f)(1):   Warning lines were not erected around all sides of the roof work area:  On or about May 28, 2013, west edge of roof, the employer did not ensure warning line was completely erected around all sides of the roof area where superintendent/monitor and employees were installing new duro-last roof over existing metal roof. An employee was observed working in area of roof where the warning line was touching the roof. This created a condition which exposed employees to unprotected falls greater than 18 feet to ground below.

29 CFR 1926.502(h)(1)(v):   The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function:   On or about May 28, 2013, at the center of the low-sloop roof, the employer did not ensure that the supervisor/safety monitor was not distracted from his monitoring activities.    Instance a:  Supervisor/Safety Monitor was bent over helping employees install roofing components to new duro-last roof system.  This created a condition which exposed employees to unprotected falls greater than 18 feet to ground below.   Instance b: Supervisor/Safety monitor was greater than 6 feet away from employee with his back turned, while the employee was in close proximity to the west roof edge of downed warning line. This created a condition which exposed employees to unprotected falls greater than 18 feet to ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about May 28, 2013, the employer did not ensure the aluminum extension ladder extended three feet above the roof landing.  An employee was observed using the ladder to access the roof while performing roofing activities of an education building.  This condition exposed the employees to the hazard of stumbling and falling from heights greater than 18 feet to the ground below.

29 CFR 1910.1200(g)(8):   The employer shall maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and shall ensure that they are readily accessible during each work shift to employees when they are in their work area(s). (Electronic access and other alternatives to maintaining paper copies of the safety data sheets are permitted as long as no barriers to immediate employee access in each workplace are created by such options.)  On or about May 28, 2013, at the jobsite, the employer did not provide hard copies of safety data sheets for each hazardous chemical such as Duro-Caulk Plus [Methyl ethyl ketoxime (MEKO)] and Acetone and make them readily accessible at the jobsite for immediate access without barriers such as, but not limited to, electronic access and at the main office.
