29 CFR 1926.451(b)(6): On scaffolds where scaffold planks are abutted to create a long platform, each abutted end shall rest on a separate support surface. This provision does not preclude the use of common support members, such as "T" sections, to support abutting planks, or hook on platforms designed to rest on common supports.      Location: 1100 West National Road, Englewood, OH    On or about 30 May, 2013    a) On the lower level of tubular welded scaffolding on the south end of site employees were observed working on tubular welder scaffolding that had wooden decking, used to support both personnel and materials, that were overlapping and not supported by structural members in that decking form one section was resting directly on decking from another section, forming a "T" junction.    b) The second level of tubular welded scaffolding on south end of site employees were observed working on tubular welder scaffolding that had wooden decking, used to support both personnel and materials, that were overlapping and not supported by structural members in that decking form one section was resting directly on decking from another section, forming a "T" junction.       c) The second level of tubular welded scaffolding in the center of the site employees were observed working on tubular welder scaffolding that had wooden decking, used to support both personnel and materials, that were overlapping and not supported by structural members in that decking form one section was resting directly on decking from another section, forming a "T" junction.

29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold shall be provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. When the falling objects are too large, heavy or massive to be contained or deflected by any of the above-listed measures, the employer shall place such potential falling objects away from the edge of the surface from which they could fall and shall secure those materials as necessary to prevent their falling.    Location: 1100 West National Road, Englewood, OH     On or about 30 May, 2013    Employer did not protect employees from potential falling materials that were stacked on tubular welding scaffolding in that concrete blocks were stacked on the edge of scaffold decking while employee worked on or below the decking.

Location: 1100 West National Road, Englewood OH    On or about 31 May, 2013    a) Employees were observed working on tubular welded scaffolding that was not adequately supported in that base plates were not installed on 13 sections of the scaffolding that were in use.
