29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Fabrication shop - A site specific lockout program including written procedures and employee training was not developed for employees who serviced and set up production equipment including, but not limited to the Brambana CNC machines, the Brambana Antea 750 saw, and the Mantello Antonino Saw. On or about 6/7/13. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

On or about 6/13/13. ABATEMENT NOTE: Frequent inspections shall be conducted on daily to monthly intervals, and shall include the items specified in 1910.179(j)(2)(i - vii). NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
