29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts: a) On or about June 5, 2013, at the above located worksite, employees that worked in the kitchen were required to use a food service conveyor (Hussman Foodservice Company, Food Service Conveyor, Model TM-10-SP-VB-NW-23) with broken slats. The broken slats resulted in unguarded pinch points and exposed the employees to the hazards associated with rotating parts.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not used or installed in accordance with instructions included in the listing or labeling: a) On or about June 5, 2013, at the above located workplace employees were required to work at a UL Listed food service conveyor (Hussman Foodservice Company, Food Service Conveyor, Model TM-10-SP-VB-NW-23) which was missing on/off switches and/or cover plates on the east and west ends of the conveyor. Without the switches and/or cover plates the conveyor was not being used in accordance with instructions included in the UL listing.
