29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    1)  Tubbing Room - The installed eyewash was not adequate in that the water flow from the two nozzles was not even so that both eyes could be flushed simultaneously.   2)  Press Room A - The installed eyewash was not working - there was no water flow when activated.

29 CFR 1910.217(c)(2)(i)(b): Point of operation guard(s) on mechanical power press(es) did not conform to the maximum permissible opening(s) specified in Table O-10:  Employees were exposed to amputation hazards while operating the following mechanical power presses where the discharge openings in the guards did not meet the requirements in Table O-10:  1)  Press Room A - Perkins 35 ton with 1498 tooling:  The opening was approximately 5-inches x 3.5-inches and the distance from the point of operation was approximately 8-inches;  2)  Press Room B - V&O Press 3:  The opening was approximately 5.25-inches x 5.5-inches and the distance from the point of operation was approximately 9-inches;  3)  Press Room B - Minster B1-32, No. 19:  The opening was approximately 8.25-inches x 5.5-inches and the distance from the point of operation was approximately 9-inches;  4)  Press Room B - Unknown Press, No. 36:  The opening was was approximately 3-inches x 10-inches to the left of the discharge feed, and approximately 6-inches x 4-inches above the discharge feed, and the distance from the point of operation was approximately 15-inches.
