(a) On or about May 30, 2013 the inspection revealed that employees were potentially exposed to serious injury from electrical shock or cuts from lights that were not protected from accidental contact and breakage. Lights were located in the tunnel under the stock out tower at center east 1, located at the chip terminal at 66237 Jordan Cove Road in North Bend, Oregon.

(a) On or about May 30, 2013 the inspection revealed a junction box that was corroded with an open front cover. The junction box was located about 25 feet west of the belt five tail drum area at the chip terminal located at 66237 Jordan Cove Road in North Bend, Oregon.
