29 CFR 1910.26(c)(3)(iv): The top of the ladder was not placed with the two rails supported, unless equipped with a single support attachment: a) On or about 05/30/2013 at 201 Gulf of Mexico Dr. in Longboat Key, Florida, an employee was observed standing on a Werner extension ladder that had the two rails unsupported and exposed to a fall hazard of approximately 7 feet.
