St. Marys Carbon Company Inc.; warehouse - On or about 6/3/13, oxygen and acetylne cylinders were being stored in a cage together.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval nor the plate being changed to reflect the capacity change with the attachment: A.) St. Marys Carbon Company Inc. - On or about 6/3/13, employees were utilizing a man basket on various forklifts without receiving manufacturer's approval for the device. In addition, the capacity plate had not been changed/updated to reflect the attachment. The man basket was used for activities including but not limited to, light bulb changes.
