29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:  On or about 5/31/13 in the vehicle bay/storage area: employees are exposed to a slip hazard from the oily substance on the floor while inspecting, loading and unloading their vehicles daily. The employer failed to protect the employees from the slippery fluid on the floor in the employee work area.

29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  On or about 5/31/13 at 6265 McDonough Drive Norcross, GA 30093: the exit door at the main entrance and door leading to the exit route from the break room area was locked and required a key or someone to unlock the door for employees to exit the building safely.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about 5/31/13 at 6265 McDonough Drive Norcross, GA 30093: an energy control program with specific energy control procedures were not developed where employees was required to perform repairs, maintenance, service, and troubleshoot the vehicles. Employees were observed repairing the vehicle without locking or tagging out the vehicle.

On or about 5/31/13 at 6265 McDonough Drive Norcross, GA 30093: the employer failed to evaluate powered industrial truck operators with up to two years of experience. The operators were required to use the powered industrial truck to load and unload containers of cash.
