29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a.     On or about May 30, 2013, on the 7:12 pitch roof, employees performing siding work were not protected from an 12 feet 6-inch fall hazard by the use of a fall protection system.

On or about April 11, 2013, employees working from elevations were not trained about fall protection.

On or about May 30, 2013, employees on the roof, were using an 8-foot Werner stepladder that was folded together in order to gain access and egress from a parapet wall to the roof, exposing employees to a 6 feet fall hazard.
