29 CFR 1926.756(a)(2): A competent person did not determine if more than two bolts were necessary to ensure the stability of cantilevered members: a. Center bridge span- On or about May 30, 2013 an employee was struck by a crane rail that fell in the south machine room. Prior to the employee removing the center connection of the cantilevered member the employer failed to inspect the connections of the cantilevered member to determine whether additional support was needed to maintain stability.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a. South machine room- Employees were not protected from slip and trip hazards where safe access was not provided to each work area along the machine room. Floor grating was not installed creating breaks in elevation of up to 30 inches.
