29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  Jobsite: The employee, standing on an aluminum ladder to help another employee on a scaffold, was not protected against electrical shock by maintaining appropriate safe distance from power line, de-energizing the service power line to the house, and/or other effective means.

29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive siderails:  Jobsite: The aluminum ladder, on which and employee was standing on to help another employee on a scaffold, was placed in close proximity to the service power lines to the house without appropriate means of protection.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Jobsite: The employee, required to set up and stand on an aluminum ladder within close proximity to electrical lines, was not given the required training necessary to recognize and prevent ladder hazards.
