29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. On or about May 31, 2013, at the above addressed jobsite, an employee was observed installing/removing roofing material from a rooftop (trash collection area) without the use of conventional fall protection, or without a safety monitoring system alone.   b. On or about May 31, 2013, at the above addressed jobsite, an employee was observed installing/removing roofing material from a rooftop (storage room area) without the use of conventional fall protection, or without a safety monitoring system alone.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  a. On or about May 31, 2013, at the above addressed jobsite, an employee was installing/removing roofing material from a rooftop, for which the anchorage points for the personal fall arrest system, was not capable of supporting at least 5,000 pounds.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. On or about May 31, 2013, at the above addressed jobsite, an employee was using a portable metal extension ladder to gain access to a roof-top (trash collection area), which was not extended at least three feet above the upper landing surface.   b. On or about May 31, 2013, at the above addressed jobsite, an employee was using a portable metal extension ladder to gain access to a roof-top (storage room area), which was not extended at least three feet above the upper landing surface.
