29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees: On or about 06/13/2013 at 4680 Lexington Road, Athens, GA: An employee bolting the braces in place while working in a scissor lift was exposed to a fall of more than 19 feet to the lower level. The General Contractor failed to have the employee removed from standing on a board resting on the midrails of the scissor lift guardrail system.

The General Contractor failed to ensure the employees were utilizing fall protection.
