29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 5 Lehigh Court, Lititz PA; Roof - Two roofers observed tearing shingles off a steep pitched residential roof were not provided fall protection and were exposed to a 17 foot - 8 inch fall hazard to the ground below, on or about June 4, 2013.   No abatement certification or documentation required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) 5 Lehigh Court, Lititz PA; Roof - Roofers were not trained in appropriate fall protection methods to avoid hazards associated with elevated work areas, on or about June 4, 2013.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 5 Lehigh Court, Lititz PA - Roofers used a 28 foot Werner portable extension ladder to gain access to a roof and the ladder extended 13 inches above the landing surface exposing the workers to a 17 foot - 8 inch fall hazard to the ground below, on or about June 4, 2013.  No abatement certification or documentation required.
