29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) For the Florida Structural Steel facility at 2710 5th Avenue, Tampa, FL 33605 as observed on or about 06/04/2013: Employees in the fabrication shop were exposed to potential laceration/amputation hazards associated with an unguarded blade on the unused blade area of a horizontal metal band saw.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) For the Florida Structural Steel facility at 2710 5th Avenue, Tampa, FL 33605 as observed on or about 06/04/2013: employees in the fabrication/welding shop used a hydraulic brake press which had no guarding mechanism for the point of operation. Adequate guarding devices such as two hand trip/control, restraints, presence sensing devices, or any other guarding means were not in place to protect employees from having their hand in the danger area associated with the point of operation.
