29 CFR 1910.107(c)(6): Electrical wiring, motors, or other equipment outside of, but within twenty (20) feet of any spray area, and not separated therefrom by partitions, did not conform to the provisions of subpart S of this part for Class I, Division 2 Hazardous Locations:    (a) Lay-up Area - An employee sprayed flammable contact adhesive onto parts that were to be laminated in a spray area.  The conduit and junction box on the "Mar Bel" Pinch Roller were broken.  Wiring was exposed within 20 feet of the spray area that did not conform to the provisions of Class I, Division 2 hazardous locations on or about June 4, 2013.     (b) Lay-up Area - An employee sprayed flammable contact adhesive onto parts that were to be laminated in a spray area.  The 120 Volt A/C electrical raceway within 20 feet of the spray area on the north wall did not conform to the provisions of Class I, Division 2 hazardous locations on or about June 4, 2013.     No abatement certification or documentation required.

A six foot long guard was removed from the movable cutting head.  The employee was exposed to flying chips from the cutting tool on or about June 4, 2013.    (b) BIESSE CNC Machine - An employee operated the BIESSE CNC machine to cut out wooden parts.  The pressure mats that were intended to shut down the CNC machine when stepped on were not together nor securely fastened in place, thereby creating an unprotected area at the center of the table where the employee could bypass the guard and work near the moving parts on or about June 5, 2013. The drill press was used while on a cart with wheels.  The drill press fell over and was damaged because it was not securely anchored to prevent walking or moving.  This condition was most recently observed on or about June 4, 2013.    (b) Profile Sander - Employees used the MB Maschinebau GnbH profile sander to manufacture custom cabinetry.  The profile sander was connected to power but was still on it's shipping crate skid on or about June 4, 2013.    (c) Band Saw - Employees used the Tannewivz band saw to manufacture custom cabinetry.  The bandsaw was resting on a wooden skid but was not securely anchired to prevent walking or moving on or about June 4, 2013.    (d) Edge Sander - Employees used the Ekstrom - Carlson No112A edge sander to manufacture custom cabinetry.  The edge sander was resting on two wooden 4 X 4 timbers but was not securely anchored to prevent walking or moving, on or about June 4, 12013. 29 CFR 1910.213(f)(2): Self-feed circular ripsaw(s) were not provided with sectional non-kickback fingers for the full width of the feed rolls:    (a) Diehl #75 Straight Line Rip Saw - The kickback fingers for the Diehl # 75 straight line rip saw were bent and damaged causing them to wedge against each other and not return to the lowest position after employees ran stock through the saw.  The fingers did not function as intended to provide anti-kickback protection.  A piece of wood kicked back out of the saw with enough force to damage an insulated steel garage door 24 feet away. The reset switch box had a 1 1/2 inch X 2 1/2 inch hole in its front panel, approximately 2 inches above the reset switch and a round knockout plug was missing from the top of the box.  Employees were exposed to the live 3 phase 230 VAC current inside the box on or about June 4, 2013.    No abatement certification or documentation required.
