29 CFR 1926.102(a)(2): Eye and face protective equipment being worn by the employees did not meet the requirements specified in American National Standards Institute, Z 87.1-1968, Practice for Occupational and Educational Eye and Face Protection:     An employee was using a pneumatic nail gun to install shingles and the employee wore sunglasses which did not meet the requirements of Z 87.1-1968, and did not have side shields to protect the eyes from side exposure to flying debris.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:    Employees were exposed to a fall hazard over 25 feet while removing and  installing roofing material and no fall protection was being used.
