29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    (a) Franklin Science Building south edge of roof - An employee walked to the edge of the roof past the warning line which was lying on the roof deck to put roofing debris, into the box on the forks of a JCB rough terrain forklift.  The employee was not wearing personal fall arrest equipment and was exposed to a potential fall of 35 feet to the ground below, on or about June 5, 2013.     (b) Franklin Science Building north edge of the roof - Employees walked along the roof near the edge to retrieve roofing material that was stored at the west end of the building's south wing.  There was no warning line established in this area and the men did not use personal fall arrest equipment and were exposed to a potential fall in excess of 35 feet to the ground below, on or about June 5, 2013.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area:     (a) Franklin Science Building Roof - The employer did not ensure that the warning line was erected around all sides of the work area. Employees performed roofing work and were required to walk to various areas to get materials and supplies, they were exposed to a 35 foot fall hazard from the roof to the ground below, on or about June 5, 2013.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Men performed roofing work in the area adjacent to the warning line lying on the roof deck and were exposed to a potential fall of 35 feet to the ground below, on or about June 5, 2013. 29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:    (a) Franklin Science Building, roof -The employer did not establish a material handling area that was connected to the work area by two parallel warning lines, where the JCB 550 170 rough terrain forklift was positioned at the roof edge.  An employee loaded roofing debris into the wooden box on the forks of the JCB forklift. The employee was exposed to a potential fall of 35 feet to the ground below, on or about June 5, 2013.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.    (a) Franklin Science Building South PARKING AREA - The employer positioned the JCB 550 170 rough terrain forklift on the sidewalk with the boom elevated to the roof edge and left the machine unattended.  A wooden box was secured to the forks and men on the roof placed construction debris into the wooden box, on or about June 5, 2013.       No abatement certification or documentation required.
