The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:    Employees removing and installing roofing material were exposed to a fall hazard up to 19 feet and the employees were not provided training which would have enabled them to recognize the hazards of falling and provided them with the procedures to follow to minimize the hazards.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:      Employees were exposed to electrical hazards due to the lack of strain relief on the black cord supplying power to a portable generator. The cord was hanging and not secured.
