29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: (a) Firestone Liquors at 562 1st St., Firestone, CO 80520: On and before June 3, 2013, the employer failed to provide a workplace free from recognized electrical hazards in that the flexible power cord on the water heater had exposed inner insulation. This condition exposed the employees to the hazard of electrical shock.

29 CFR 1910.303(g)(1)(i): Workspace was less than 3 feet in front of electric equipment operating at 600 volts nominal, or less: (a) Firestone Liquors at 562 1st St., Firestone, CO 80520: On and before June 3, 2013, the employer did not maintain workspace of at least 3 feet in front of an electrical circuit breaker panel in the back storage room in that it was obstructed by product that was stacked in front of it. This condition exposed the employees to electrical hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) Firestone Liquors at 562 1st St., Firestone, CO 80520: On and before June 3, 2013, the flexible power cord on the water heater was not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws which exposed the individual conductors underneath the cord jacket at the point where the wiring entered the appliance. This condition exposed the employees to the hazard of electrical shock.
