29 CFR 1926.451(d)(3)(vii): Tiebacks shall be equivalent in strength to the suspension ropes:   a) On or about May 28, 2013, at the jobsite located at 1034 S. Kostner Avenue, Chicago, Illinois - employees were performing masonry activities from a two point suspended scaffold approximately fifty (50) to sixty (60) feet above the ground. The rope used to tieback the scaffold was inadequate and improperly installed, thereby exposing employees to fall hazards.   b) On or about  June 03, 2013, at the jobsite located at 1034 S. Kostner Avenue, Chicago, Illinois - employees were performing masonry activities from a two point suspended scaffold approximately fifty (50) to sixty (60) feet above the ground. The wire rope used to tieback the scaffold was inadequate and improperly installed, thereby exposing employees to fall hazards.

29 CFR 1926.451(d)(16): In addition to the normal operating brake, suspension scaffold power-operated hoists and manually operated hoists shall have a braking device or locking pawl which engages automatically when a hoist makes either of the following uncontrolled movements: an instantaneous change in momentum or an accelerated overspeed:    a) On or about May 28, 2013, at the jobsite located at 1034 S. Kostner Avenue, Chicago, Illinois - employees were performing tuckpointing operations from a two point suspension scaffold, approximately fifty (50) feet above ground, that was not provided with a braking device or locking pawl which engages automatically following a uncontrolled movement, thereby exposing the employees to fall hazards.

Kostner Avenue, Chicago, Illinois - employees were performing tuckpointing operations from a two point suspension scaffold, approximately fifty (50) feet above ground, equipped with a manually operated hoist. The hoist did not require a positive crank force to descend, thereby exposing the employees to fall hazards.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about May 28, 2013, at the jobsite located at 1034 S. Kostner Avenue, Chicago, Illinois - employees performing masonry activities on the roofs were exposed to various fall hazards of approximately sixteen (16) to thirty (30) feet created by the presence unprotected sides and edges.

Kostner Avenue, Chicago, Illinois - employees performing masonry activities in the building were working near unprotected wall openings with fall hazards of approximately ten (10) to fifty (50) feet, thereby exposing the employees to fall hazards.   b) On or about May 28, 2013, at the jobsite located at 1034 S. Kostner Avenue, Chicago, Illinois - employees performing masonry activities in the building working near unprotected interior elevator shafts/openings were exposed to fall hazards of approximately ten (10) to fifty (50) feet.
