29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a)  On or about May 22, 2013, and at times prior, the employer did not ensure that employees were protected from shock, burn and fire hazards in that strain relief was not provided on the hanging pendants in the repair shop:   1) the insulation was pulled back from the junction boxes of the hanging pendant for bay 1.  2) pendants in bays 1-4 were hung by the cords with no strain relief mechanism to prevent pull from damaging the cord.    (b)  On or about May 22, 2013, and at times prior, the employer did not ensure that employees were protected from shock, burn and fire hazards in that the insulation on each of three extension cords at the block heater plug station and one in the shed, was broken and pulled back from the plug.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
