29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:    a) On 6/4/13, the planks forming the working surface of the Hydro-Mobil scaffold had shifted and gaps greater than 1 inch wide were created in multiple places.

29 CFR 1910.178(l)(3)(i)(G): Powered industrial truck operators did not receive initial training on fork and attachment adaptation, operation, and use limitations; Construction reference: 1926.602(d):    a) On 6/4/13, a rough terrain forklift operator suspended and placed a mortar pan on the Hydro-Mobil scaffold without an appropriate attachment and had not been trained in the proper use of attachments for suspending loads from a forklift.
