29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) Side of 4640 W. Tilghman Street, Allentown, PA - On or about June 5, 2013, an employee was working from a Baker Vehicle mounted aerial lift, at a working height in excess of 14 feet above grade, without a body belt/personal fall arrest harness with an attached lanyard being worn and attached to the boom or basket.
